The Gravity Forms Survey Add-On<\/a> makes it quick and easy to create a survey. One of our most popular add-ons, Survey seamlessly integrates with Gravity Forms enabling you to interact with your users to both collect and analyze data.<\/p>\n The Survey Add-On adds a Survey field to your form editor – within this Survey field, there are 8 Survey Field Types from which you can create your questions.<\/p>\n Let\u2019s take a look at some of the most popular and engaging options that you can choose from\u2026<\/p>\n To get started, you first need to install the Survey Add-On. To do so, within your WordPress dashboard, select Forms > Add-Ons<\/strong> from the left-hand menu. Then scroll down the add-on page to Survey<\/strong> and click on Install<\/strong>.<\/p>\n Next, click on Forms > Add New Form<\/strong> to open a new form – you will need to give it a name and a description (optional). Equally, once installed, the Survey Add-On can be used in conjunction with any pre-existing form.<\/p>\n Before you start adding survey questions, you\u2019ll probably want to populate your form with some basic fields to help you identify your participants. These fields will differ depending on the type of survey you are creating, but at the very least, you should add the Name<\/strong> and Email<\/strong> fields.<\/p>\n Now it\u2019s time to make your survey! Add a Survey<\/strong> field from the bank of Advanced<\/strong> fields on the right.<\/p>\n Insert your question in the empty space beneath Survey Question<\/strong>. Then under Survey Field Type<\/strong>, select the type of survey question you want to use.<\/p>\n The Columns\/Choices<\/strong> section of the survey field is where you input the selections for whichever Survey Field Type<\/strong> you choose. Use the plus and minus icons on the right to add or remove options.<\/p>\n Before completing the field, browse the Appearance<\/strong> and Advanced<\/strong> tabs at the top of the field editor. Like most Gravity Forms fields, you can enable conditional logic<\/a> and dynamically populated data<\/a>.<\/p>\n For each new survey question you will need to add a new Survey<\/strong> field – once you have finished click Update<\/strong> to save your form. Head over to our Form Template Library<\/a> to download our pre-built survey template<\/a>, then simply import it to your website\u2026<\/p>\n Once imported, this form can be easily customized to suit your exact needs.<\/p>\n Before you can add your survey to a page and make it available to participants, you’ll want to configure a few settings that apply to the entire form. You\u2019ll find them in Settings<\/strong> at the top of the form builder.<\/p>\n Below we will discuss Confirmations, <\/strong>and Notifications<\/strong>, but if you want more information on general Form Settings<\/strong>, check out the docs<\/a>.\u00a0<\/strong><\/p>\n Under Confirmations<\/strong>, you can edit the on-page message users see after they\u2019ve submitted the survey. You\u2019ll want to change the default confirmation to something like \u201cThanks for completing the survey! We\u2019ll get back to you shortly.\u201d<\/p>\n For more information, have a read of Configuring Confirmations<\/a>.<\/p>\n Under Notifications<\/strong>, you can change or add emails that WordPress sends out when someone submits a form. The default admin notification will send a copy of the responses to the WordPress administrator. We also recommend creating a notification that sends an email to the survey participants with their responses or to deliver their incentive.<\/p>\n Again, for more information, have a read of the docs – Configuring Notifications.<\/a><\/p>\n Before people can take your survey, you\u2019ll have to embed it to a web page. This is a quick task. It\u2019s just like embedding any Gravity Form.<\/p>\n You can use the Gravity Forms Block in the Gutenberg editor (recommended) or add the shortcode in the Classic Editor. Read our guide on Adding a Form to Your Site<\/a> to help you with this.<\/p>\n Once your participants have had some time to take the survey, view the results by clicking the Survey Results<\/strong> tab at the top of the form editor.
